Alumni (and Non-enrolled Students)
Please download and complete the
Alumni Transcript Request Form using the link below.
Return the form with your release signature and $5.00 per transcript to the Principal's Office at
Regis High School, 55 East 84th Street, New York, NY 10028, Attention: Transcripts.
Allow 2 to 3 days for processing from time of receipt.

Students: Grades 9 - 11
During the school year, please speak to your respective Guidance Counselor about any
transcript requests. For each transcript you need, please complete a
Transcript Request
Form (Current Students - Non-senior), and return the completed form with your signature
release directly to your Guidance Counselor. Please submit your request to your guidance
counselor at least two weeks prior to your deadline.

Important Note: After June 14th until the start of the new academic year, download and
complete the Transcript Request Form using the link below. Return the form with your release

Students: Grade 12
During the school year, please speak to your respective Guidance Counselor about any
transcript requests. For scholarship and incidental transcript requests please complete a
Transcript Request Form (Senior), and return the completed form with your release signature
directly to your Guidance Counselor. Please submit your request to your guidance counselor at
least two weeks prior to your deadline.
Seniors are billed $2.00 per transcript.

All transcripts for college applications will be handled separately. For graduating seniors,
the Director of College Counseling will handle the mailing of all final forms and transcripts. All
final transcripts will be sent by college group, and via a "trackable" method.
General questions
about final transcripts may be directed to the Director of College Counseling through June 28th.
Important Note: From June 29th to August 27th, any special transcript requests or questions
may be directed to the Principal's Office. Additional transcript requests after graduation should
be accompanied with a payment of
$5 per transcript and should be submitted in writing to the
Principal's Office with an Alumni Transcript Request Form located on this internet page.
